  • after submitting the form, you will receive an email asking confirmation of your signature

  • after confirmation, we will both receive a completed form over email

• required for all NEW students

  • if you want to practice with friends, each person must fill out a form

• NO exceptions and one-time only

• hands-on adjustments offered with proof of full vaccination
